    Differences that matter:Understanding case-mix and quality for prospective payment of home care

    A suitable payment system is necessary to provide efficient, high-quality care at home. Therefore, in 2017, the Dutch Healthcare Authority initiated a project to develop a new home care payment system based on client profiles. This means a budget per client is determined beforehand, which suits a client’s expected care use. To do so, insight is needed on which client characteristics can predict the expected home care use. This was studied in this dissertation, by asking district nurses and by studying scientific literature. Among others, client characteristics related to a client’s functioning in daily life – for example if a client is independent in showering or using medication – were considered as relevant predictors of home care use. Subsequently, with this knowledge the Case-Mix Short Form was developed, tested and implemented, together with district nurses, home care providers and healthcare insurers, to collect data on relevant predictive client characteristics in a standardized manner. Finally, this dissertation provides an exploration of suitable outcomes for quality measurement in home care

    Relations entre feuilletons documentaires et choix de métier chez les adolescents

    L’objectif de cette étude est d’explorer, chez les adolescents, la relation entre le fait de regarder des feuilletons documentaires et leurs projets de métier. Les résultats montrent que, parmi un échantillon de 369 adolescents en dernière année d’enseignement secondaire, le fait de regarder régulièrement un feuilleton documentaire portant sur les sages-femmes, les vétérinaires ou les parachutistes a été un prédicteur significatif d’une perception plus positive de cette profession. Des données écologiques sur les effectifs d’étudiants suggèrent que ceux-ci ont augmenté au cours des périodes qui ont suivi la diffusion d’un feuilleton documentaire à succès portant sur des professions particulières. La probabilité de s’inscrire comme étudiant dans un métier récemment montré dans un feuilleton documentaire a augmenté de 15 %. La discussion se centre sur les implications de ces résultats et met l’accent sur la nécessité de continuer des recherches dans cette sphère.This study aimed to explore the relationship between adolescents’ viewing of docu soaps and their career aspirations. The results show that among a sample of 369 adolescents in their last year of secondary education, regular viewing of a docu soap about midwives, veterinarians or para-commandos was a significant predictor of more positive perceptions of that profession. Ecological data on student numbers suggested that these went up in periods following the broadcasting of a successful docu soap about a particular profession. The odds of becoming a student of a study programme recently featured in a docu soap increased by 15 percent. The discussion focuses on the implications of these findings and emphasizes the need for continued research in this area
